Request it by contacting our team. For fastest service, include:
- Full name + date of birth
- Date of visit (or date range)
- What you need (visit note, lab results, immunization record, work/school note, etc.)
- Where to send it (you, another clinic, employer/school—if applicable)
Sending records to another provider
We can help coordinate transfer—share the receiving provider’s name and contact details when you reach out.

Insurance & Coverage
We work with a range of insurance providers, including:

- Atlantic Medical (CG Atlantic)
- Bahama Health
- British America Financial (BAF) Insurance
- First Care
We also accept most commercial plans. To keep your visit smooth:
- Bring your insurance card and a valid ID
- If you’re unsure about coverage, confirm benefits with your insurer (your carrier may request our tax ID)
Financial Policy
Patients are responsible for all charges incurred, including co-pays, co-insurance, deductibles, non-covered services, missed appointments, and any additional costs related to collections if necessary. If your insurance plan requires a designated primary care provider and that provider is not affiliated with Stellar Urgent Care Clinic, you may be responsible for any out-of-network charges.
Please note that regardless of insurance coverage, you are responsible for the full payment of all fees for services provided by Stellar Urgent Care Clinic.For your convenience, we accept the following forms of payment:
